V/Line’s organisational processes for responding to and rectifying rail creep defects did not ensure that all such defects were addressed in a timely way.
The safety issue status remains pending as V/Line implements its new track lateral stability standard, and ATSB considers any related findings from ongoing investigations RO-2015-022 (Nunga) and RO- 2015-029 (Ouyen).
V/Line has advised that it will review its processes and timeframes for addressing C2 defects. This review is to be completed by November 2015.
V/Line has advised that it has completed its review and update of its track standard for the inspection and assessment of lateral stability and has indicated that a plan for implementation of the standard will be completed by October 2016.
