The documentation and quality control processes used by the South Improvement Alliance for the Cootamundra re-signalling project were not sufficiently robust, in particular, the closing out of identified design issues was inadequate.
THe ARTC has introduced additional verification processes to record design changes changes in the works. The ATSB has reassessed the risks associated with this safety issue and while there is still a risk of injury, the ATSB believes the responses provided by the ARTC and RailCorp adequately address this safety issue.
