Bowen Tug and Barge did not have an effective compliance auditing process in place to ensure that its employees were following the training they had received and the guidance contained in the safety management system documentation.
Minor safety issue for which the company reviewed its operations, altered how it handled the containers and then divested itself of its interests.
Bowen Tug and Barge advised that the company has undertaken a continuing review of its operations and work practices including those associated with this accident.
Further, as of 01 July 2012 the company was no longer in the business of loading and unloading of stores for ships and was selling all the associated equipment to another company.
The ATSB is satisfied that the actions taken will reduce the likelihood of a similar accident occurring within this company in future.
