Bowen Tug and Barge’s safety management system guidance for barge storing operations did not designate roles or responsibilities to specific individuals and a system for communicating with the ship’s crew was not discussed and established.

Review of operation and work practices; sold company operations Bowen Tug and Barge advised that the company has undertaken a continuing review of its operations and work practices including those associated with this accident.
Further, as of 01 July 2012 the company was no longer in the business of loading and unloading of stores for ships and was selling all the associated equipment to another company.
The ATSB is satisfied that the actions taken will reduce the likelihood of a similar accident occurring within this company in future.
