Action description

The ATSB has been advised by Sea Swift that the company has reviewed all emergency release mechanisms associated with it’s tug and barge operations. This has been conducted in association with a directive issued by Maritime Safety Queensland relating to the safe operation of the tow hook on all Queensland registered tugs, following this casualty. This review resulted in each tug's ‘Safe Work Procedure’ for tow hook operations and emergency release being updated. The tug crews are now required to observe and operate these releases under training and during regular testing in accordance with Sea Swift’s annual drill schedule.

All personnel are required to acknowledge their understanding of and competence in the operation of this release and the tow hooks operation and each entry in the ‘Safe Work Procedure’ is required to be verified by the tug’s master.
