The pre-flight safety briefing and safety information card did not include a clear instruction on how to activate the flow of oxygen from the passenger oxygen masks and that the bag may not inflate when oxygen is flowing. This resulted in some passengers not understanding whether or not there was oxygen flowing in the mask.
AirAsia Indonesia have implemented changes in accordance with the safety issue raised by the ATSB. This included amending its passenger safety briefing cards and passenger announcement to include the instruction that if oxygen masks deploy the passengers must pull down on them firmly, and that the oxygen mask bags may not inflate during use. The action taken is considered to adequately address the safety issue.
The ATSB recommends that AirAsia Indonesia take further action to review its current passenger pre-flight safety briefing and safety information card to ensure passengers are provided with clear instruction on how to activate the flow of oxygen from the passenger oxygen masks and that the bag may not inflate when oxygen is flowing.
Indonesia AirAsia has agreed and we are in the process of responding to the safety recommendation as follows:
1. We will revise our A320 Safety Instruction Card to ensure passengers are provided with additional information in order to better understand how to use their oxygen mask in an emergency. It will be implemented by 01 April 2020 (see the attached SIC sample).
2. We will revise our A320 pre-flight safety briefing to ensure passengers are provided with additional information in order to better understand how to use their oxygen mask in an emergency. Since the pre-recorded type is requiring a new recording and installation for all AirAsia group fleets, we will implement the revision part of announcement by disseminating a special notice to Cabin Crew, and expect to be implemented by 01 March 2020 (will be sent soonest the Notice disseminated).
As previously mentioned, please find enclosed FON (Flight Operations Notice) regarding the additional announcement to the existing passenger preflight safety briefing.
The said Safety Information Cards (SIC) have been implemented and distributed on the aircraft. Distribution was conducted on April 27 as stated in the email attached. Please find attached email as the sample for further information regarding SIC distribution to aircraft.
