The aircraft operator did not comply with the reporting requirements of the Transport Safety Investigation Act 2003.
The operators safety management system contents have been modified to better suit the reporting of incidents and additional training has been provided to those responsible for reporting incidents within the airline.
Action taken by the aircraft operator The aircraft operator has advised that in response to this incident, the content of the operator’s safety management system in respect of incident reporting has been revised, and additional training in that regard has been incorporated into the operator’s cabin and flight crew induction processes. ATSB assessment of response/action The ATSB is satisfied that the action taken by the aircraft operator adequately addresses the safety issue.
