The ATSB is investigating an occurrence where the aircraft took-off with an incorrect configuration involving a Bombardier DHC-8-402, VH-QOI, at Horn Island Airport, Queensland, on 26 June 2024.
During the pre-flight preparation, the crew inadvertently selected flap 5 for departure instead of the flap 15 setting resulting in a departure with an incorrectly configured aircraft.
The evidence collection phase of the investigation will involve interviewing involved parties, retrieving and reviewing recorded data, and the collection of other relevant information.
A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
