The ATSB is investigating two separate runway overruns on take-off involving an Airbus A330, registered 9M-MTL, on 7 September 2023, and a Boeing 787-9, registered VN-A819, on 18 September 2023, at Melbourne Airport, Victoria.

At the time of each occurrence, runway 34 at Melbourne Airport was temporarily shortened for runway overlay works. During the respective night take-offs from runway 34, each aircraft reportedly rotated past the temporary displaced threshold resulting in jet blast affecting people and objects and people in the work area. There were no reported physical injuries. There was no damage reported from the 9M-MTL occurrence, and several unserviceability lights were damaged by the jet blast of VN-A819.

In response to the first occurrence, Melbourne Airport temporarily ceased the runway works to investigate the circumstances of the event, review its existing risk assessment and conduct additional stakeholder communications. In response to the second event, Melbourne Airport cancelled the remaining active works incorporating reduced runway length operations. 

The ATSB has commenced the examination and analysis of the initial evidence collected. To date, the ATSB investigation has:

  • interviewed the flight crews
  • reviewed flight planning information for each flight
  • reviewed initial investigation reports from the aircraft operators and airport operator
  • reviewed the works planning documents 
  • reviewed NOTAM and ATIS information
  • analysed recorded flight data
  • reviewed CCTV footage
  • analysed recorded ATC audio and surface movement radar data
  • conducted an initial review of regulations, requirements and recommended practices for displaced threshold runway works
  • conducted a review of related occurrences
  • reviewed runway works planning and risk assessments 
  • reviewed mechanisms for the communication of safety-critical aeronautical information to air crews.

In the course of the investigation, the ATSB has determined there to be a reasonable likelihood of limitations in organisational factors relevant to the occurrence. Examination of these factors represent an increase in the scope of this investigation, and it has been upgraded from Defined to Systemic as a result (the ATSB's different levels of investigation are detailed here).

The continuing investigation will include further analysis of the evidence already collected as well as:

  • the effectiveness of regulations, requirements and recommended practices for reduced runway length operations involving runway works.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
