The procedure for predeparture testing, as part of the coupling procedure, required two competent staff. There was no procedure in the operations manual to ensure that a competent and qualified person was present to assist the driver.
The ATSB is satisfied that the updated Railway Operational Procedural Manual will reduce the risk of this safety issue.
RTA Weipa Pty Ltd advised that the Railway Operational Procedures Manual has been updated to refer to, and require, a competent person to be present to assist the driver in the coupling and uncoupling procedure.
