Safety Issue
Network rules and procedures were deficient with respect to
ensuring that back-up communication details were routinely recorded
on train control graphs to allow for a seamless transfer in the
event of primary communications failure.
Safety Advisory Notice
The Australian Transport Safety Bureau advises that all track
managers in New South Wales should consider the safety implications
of this safety issue and take action where considered
appropriate.
