existence of an error reporting policy as a positive safety
indicator.
CASA acknowledges the intent of this recommendation, and notes
that the presence of an error reporting policy may not guarantee a
measurable improvement to safety, or be viewed as a positive safety
indicator.
CASA believes, however, that the Authority's challenge is to
encourage the aviation industry to implement an error reporting
policy that focuses on systems and future improvements and is not
punitive.
