The Australian Transport Safety Bureau (formerly the Bureau of
Air Safety Investigation) recommends that the United Kingdom Civil
Aviation Authority review Rolls Royce RB211-524D4 engine cold
stream nozzle inspection criteria to minimise the possibility of
failure during operation.
The Authority accepts this Recommendation.
As a result of this event, the Authority has required mandatory
inspection of affected cold nozzles. This has been promulgated by
CAA Airworthiness Directive (AD) and Rolls-Royce Non-Modification
Service Bulletin (NMSB) 78-C955. This AD requires a fleet
inspection of all affected RB211-524D4 cold stream nozzles within
100 cycles or 400 hours (whichever limit is reached first). The
results of these inspections will be used to determine the need for
any further action.
