The Australian Transport Safety Bureau (formerly the Bureau of
Air Safety Investigation) recommends that the United Kingdom Civil
Aviation Authority notify all operators under their jurisdiction
using Rolls Royce RB211-524D4 or similar engines, of the
possibility of failure of the cold stream nozzle during
operation.

The Authority accepts this Recommendation.



Rolls Royce with the agreement of the Authority, issued a telex to
all affected operators on the 7th January 2000. The telex informed
operators about the incident, the area of concern and the means to
commence their own inspections until a non modification service
bulletin (NMSB) was issued.



Mandatory inspection has now been put in place by CAA Airworthinees
Directive (AD) and Rolls Royce NMSB 78-C955 for affected
RB211-524D4 engines.



The Authority has asked Rolls Royce to review other engine designs
with respect to this problem.
