Safety Issue
The fitting of gasket discs in an open ended cap to blank off a
fuel pipe was ineffective for the task because the discs probably
became looser due to the effect of pressure pulses within the fuel
pipe, allowing fuel to spray into the engine room where it was
ignited on the hot surfaces of the engine.

The Australian Transport Safety Bureau advises that the owners
and operators of ships should consider the safety implications of
this safety issue and take action when considered appropriate.
