The ATSB is investigating 2 runway events involving Virgin Australia Boeing 737 aircraft, VH-IJR and VH-VOL, at Adelaide Airport, South Australia, on 11 April 2025.

During take-off from runway 23 with a displaced runway start and threshold, the flight crews misidentified the location of the runway start and commenced take-off from the displaced threshold, beyond the runway start position entered in the flight management system.

The evidence collection phase of the investigation will involve interviewing involved parties, examination of operational information, reviewing recorded data, and the collection of other relevant information.

A final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify relevant parties, so that appropriate safety action can be taken.
