The ATSB is investigating a collision with terrain involving a Bell 206B-1, registration VH-ZDI, about 5 NM south-east of Tumbarumba, New South Wales, on 16 July 2023.

During manoeuvring immediately after lift-off, the pilot reported vibrations and a loss of directional control. The pilot attempted to arrest the helicopter's rotation airborne; however, they could not do so. They then attempted to land the helicopter. During the landing the helicopter rolled over, sustaining substantial damage.

The ATSB has completed the evidence collection and analysis phases of the investigation. To date, the ATSB investigation has included:

  • interviewing the flight crew and assessing training
  • examination of available site photography
  • examination of maintenance and pilot records
  • analysis of the weight and balance of the helicopter
  • examination of the wreckage at a secure facility
  • analysis of operational documentation, maintenance records, and related occurrences.

The final report has been drafted and is undergoing internal review to ensure the report adequately and accurately reflects the evidence collected, analysis, and agreed findings.

The final report will be released at the conclusion of the investigation. Should a critical safety issue be identified during the course of the investigation, the ATSB will immediately notify the relevant parties, so that appropriate safety action can be taken.
