The ATSB is investigating a level crossing irregularity involving freight train 1MP9 at Torrens Road, Ovingham, South Australia on 7 December 2020.

At about 1157 (CDT), the Torrens Road level crossing active warning equipment did not operate for the approach of SCT freight train 1MP9. The driver of train 1MP9 braked and sounded a warning but the train traversed the level crossing while the flashing lights and boom barriers were inoperative. There was no collision with any road vehicles.

The draft report internal review process has been completed. The draft report has been distributed to directly involved parties (DIPs) to check factual accuracy and ensure natural justice. Any submissions from those parties will be reviewed and, where considered appropriate, the draft report will be amended accordingly.

Following the external review process, any submissions and amendments to the draft report are internally reviewed. Once approved, the final report is prepared for publication and dissemination and released to DIPs prior to its public release. 

The final report will be published at the conclusion of the investigation. Should any safety critical information be discovered at any time during the investigation, the ATSB will immediately notify operators and regulators so appropriate and timely safety action can be taken.
