An investigation is underway into an overspeed through a set of points, BN318, between Doonside and Blacktown on 21 April 2024.
At approximately 1436, Sydney Trains service 805K traversed points BN318 at a speed of 99 km/h, where the turnout speed was 25 km/h. The driver was reportedly thrown from their driving seat, however there were no reports of injured passengers. The driver was removed from safeworking duties while investigations are undertaken.
The investigation scope includes risk controls for overspeed, condition of rollingstock and track infrastructure, train operations and scheduling, the train crew, communications and gathering any other relevant information.
This investigation is being led by the Office of Transport Safety Investigations (OTSI). OTSI conducts rail investigations in NSW under the Transport Safety Investigation Act 2003 in accordance with a collaboration agreement with the ATSB.
A final report will be released at the conclusion of the investigation. However, should a critical safety issue be identified during the course of the investigation, OTSI and the ATSB will immediately notify relevant parties, so that appropriate and timely safety action can be taken.
