On the 5 November 2022, an Airborne Australia Edge XT-912-L collided with terrain near Mokuleia, Hawaii. The aircraft was substantially damaged, and 2 people on board were fatally injured.
As the aircraft is Australian manufactured, the United States. National Transportation Safety Board requested that the ATSB appoint an accredited representative to assist in their investigation.
To facilitate this support and to provide the appropriate protections for any information gathered, the ATSB appointed an accredited representative in accordance with paragraph 5.23 of ICAO Annex 13 and commenced an investigation under the Australian Transport Safety Investigation Act 2003.
